We are providers for most insurance plans including Medicare. Please confirm that we are providers for your insurance when you schedule your appointment. Please bring the insurance card(s) of your primary (and secondary) insurance. If a referral is required by your insurance, please be sure that this has been arranged prior to your appointment. “Co-pays” are required at check-in and must be paid as required by insurance regulations. Please bring any tests or X-rays related to your ENT problem to your appointment.
